Evaluation Tools

Two sets of evaluation tools are available for those offering the SEEDS program:  1) fidelity checklists for each lesson (facilitator and observer forms) for describing the fidelity of program delivery; and 2) participant pre-post questionnaires for evaluating the influence of the program on food parenting practices and feeding self-efficacy.  The pre-post questionnaires are shortened versions of the Food Parenting Inventory1 and feeding self-efficacy questionnaires used in the initial SEEDS randomized controlled trial (RCT).2  The fidelity checklists were used in the RCT as well.  All evaluation materials are available as PDF files.  Scoring instructions are included.
